Im having this same problem , day in , day out. I stream for upwards of 3+ hours each day @ 720p and sometimes do 6+ hours. I have found i have to keep reinstalling OBS and then it works for a few days and then does the same thing as before. OBS does not crash and its saved as mp4, Maybe try and save as mkv instead ?
I am using a Nvidia GFX card with Geforce 344.11 Driver dates 18/09/2014. I am now updating to Version 344.48 , dated 22/10/2014 to see if that solves it. I will repost back if that sorts out the problem.
