Ui bugged

I hadn't opened my obs in a while and after opening it again to start recording something, I noticed the UI is totally bugged (mainly the preview screen).
When i record and check the recording it's normal but whilst recording i can't see anything due to a white screen. When resizing the window it becomes a total mess. I have tried the following fixes:
- reinstall
- try different versions
- change gpu (integrated/nvidia)
- reset settings
- update drivers
- reboot pc
nothing really seems to work so I'm puzzeled.
I have added some screenshots below for context
now added log file

Code:
17:26:32.017: device_swapchain_create (D3D11): Failed to create swap chain (887A0005)
17:26:32.017: Device Removed Reason: 00000000
17:26:32.017: obs_display_init: Failed to create swap chain
take a look at these threads (likely video driver issue, common solution is to disable integrated GPU or downgrade drivers to the times when it worked OK):

I have finally found a way to make it work, for some odd reason it does work when I have obs on my second monitor so I suggest anyone in the future to try that

HAGS is enabled, that's where I would start...

Game DVR can be disabled too.
17:26:30.194: Game DVR: On

I noticed the Nvidia Video FX plugin v0.7.2 installed is Beta. Wonder if it's not quite ready for prime time? If things don't improve after disabling HAGS, this should probably be next in line.
