hmm maybe try typing in your cmd prompt instead of ctrl+v
tasklist > tasklist.txt

and if that doesnt work:
tasklist > C:\Users\**John**\tasklist.txt

Press winkey+r this will show , type cmd there and hit Ok
cmdr.png
(or click on Start button and type cmd)
Type tasklist >tasks.txt it will look like this
cmdrtask.png

Type tasks.txt , it should start notepad.

That's exactly what I've suspected the moment I saw c:\progra~3\browse~1\261249~1.132\{c16c1~1\browse~1.dll which uses AppInit_DLL reg key to attach itself to each new process. And it's a pain in the ass to remove it.

Anyway I see you have GamerOSD running, can you close it and try again previewing in OBS ?

You should clean your PC of malware and viruses etc. OBS can't run reliably if your PC is compromised.

not sure how to clean my pc anymore than i already have, i've run various diagnostics, only other thing i can think of is spending a dozen hours in my registry.

Do this at your own risk, I'm not responsible if you break something.
In Computer management -> Services and Applications -> Services find Browser protect service , right click on it , select properties , select startup type: Disabled, apply, next click on Recovery tab and for First failure/Second failure/Subsequent failures select Take No Action, apply , ok.
Run command prompt (winkey+R cmd sau start button , type cmd) and in it type taskkill /f /im browserprotect.exe
Download and install FileAssassin from http://www.malwarebytes.org/products/fileassassin/ , run it.
Paste this c:\progra~3\browse~1\261249~1.132\{c16c1~1\browse~1.dll in the file location and make sure you select USE DELETE ON WINDOWS REBOOT FUNCTIONS. Click on execute button and reboot. Check if OBS still crashes.
PS: Also run Malwarebytes Antimalware again (quick scan is more than enough)

Progression: When I attempted to preview stream with just League set as my only source, it crashed both league and OBS. Maybe that is relevant information.

Go to c:\ProgramData\browserprotect and look for the folder that ends with 132 then find in that folder or its subfolders browserprotect.dll file, drag and drop it in fileassassin.

Please post the crash log and tasklist again.

(ProgamData folder is a hidden folder, it won't show unless you configure explorer to show hidden folders too)
