Try setting your audio devices to 44100, and turn CBR on instead of off. Might not have a huge effect, but 1) there's no good reason to make OBS convert 96000 audio to 41000, and 2) Twitch wants CBR on.

That bitrate is really high for 480p, so I don't know of much more that you can do to increase quality other than increase resolution. Try lessening the downscale (to increase resolution)? You'll probably have to stop using Medium preset if it gets much larger, but it should be more than worth the tradeoff. 2000 kbr is good for up to 720p.

Maybe something in the upper 500s or 600p? The rest I pretty much answered already. Use something like Fast or Faster preset, and the other things I said, then post a new log file of you trying it for a while.

Why it should be hard with 3770k @ 4.5 ?

Because you're gaming on streaming on the same PC? I have a 4770k and a 2600k and medium is out of reach unless its doing NOTHING else, using a capture card.

First you get it working, then you tweak settings until you go too far, then you pull back a bit. That's the right way to do it. You're going too far and then trying to tweak further. That's not the right way.

Until you heed the advice given I for one have nothing more to say.

Medium is extremely demanding on ANY modern CPU, to be frank.
With your hardware/bitrate, I'd probably go for 720@45 at 2000kbps, using the Faster or Fast preset at most.

Have you been monitoring CPU/GPU loads and temps while streaming?
Bear in mind that you will NEVER get a true 1:1 stream; some managing of expectations is usually in order for those just starting out.
