Sorry but that isn't a complete log file. Posting the link OBS generates for you would probably be easier.

Connect both monitors to your GTX960, if you're moving OBS to the second monitor while streaming and it isn't connected to the primary GPU your performance will tank. Also be sure there's some kind of frame rate limit enabled in your games so they aren't maxing out your CPU/GPU on their own.

It's as simple as it sounds: connect the cable from your second monitor to the NVIDIA GPU. It's plugged into your motherboard now.

I have no way to know what you did/what happened to your PC to erase OBS and its settings.

I have a hard time believing that your GPU is equipped with a single output. Please check again.

Well it looks like i will be switching broadcasting services since OBS decides to delete all of its info and start over again unbelievable.

OBS isn't a broadcasting service.
If you have a first-party nVidia GTX 960, it definitely has more than one port (they have five!), though you may need to buy an adapter if your monitor doesn't have the right connections for it. Usually the card will at least come with one DVI->HDMI adapter, as they're cheap.

If your system crashes, there may be some data loss. Blaming OBS would be like blaming your car for falling into the ocean when the bridge you were driving on collapsed.

720p@30fps, 2000kbps, x264 Veryfast is the recommended 'golden point' for non-partnered Twitch streamers.
