Question / Help Source permanent showing after crash, no recording possible

Hey Community, i got a frustrating bug, i hope you can help me,
after a crash of my graphic card driver this source of my last streamed game is always showing, even if it is not running.

I can stream other games, but if i choose this game while it is running i get this result, just a black screen:

Turning anti cheat on/of is not helping, the log file displays no errors. Is there a way to remove a source from the application list?

Have you tried swapping the source to Hotkey, or just deleting it outright from the sources list and creating a new one?

The Problem is that the Process in the Application list is always there even if the process is terminated, if i restart my system its still showing that the Game Plants vs Zombies Garden warfare 2 is active in the OBS Application list, so if i try to stream it, there will only be a black screen, other games work fine. I deleted "Spiel Aufnahme" and created a new game capture, still the same problem.
