Question / Help recording fails - attached logs

I have been recording on/off for 3 hrs, and during that time I only have 3 recordings-- in total 45 minutes of footage. Has the recording failed to start, or what has happened? And how can i avoid it in the future?
I never got any indication of anything not working, i just stopped and started recording multiple times, everything seemed fine until i checked my clips out...which weren't there

Here is a log file from when i pretty much did not record anything (although I blieved i recorded several segments)

You tried using flv instead of mp4 as your output format?

Also, we're past RC1 and on to full release for 0.13.1

Well i can't work with flv, i need the mp4 format, which, when it works, works great!
You suggesting that I update OBS?

Did you find anything in the logs btw of anything which could be the root cause for the failed recordings?

it says that I have OBS 0.13.1 by the way....i installed this program less than a week ago

He wants you to install the latest version of OBS, which is 0.13.1. You're currently using the release candidate build, 0.13.1-rc, which came out earlier to help test for bugs before release. Go to the front page and get the latest version of OBS Studio and install it like you normally would.
