Create another scene for your Display Capture. Having both a Game & Display capture in the same scene can cause issues.

For your Encoding settings, Use Quality & turn off Psycho Visual Tuning. The small amount of rendering/encoding lag your getting will be eliminated.
It fixed it somewhat, but now it sounds slightly too quiet and echoey.Just a couple more adjustments.

All Settings > System > Sound > Sound Control Panel > Headphones > Properties > Advanced Tab

When you changed the sample rate earlier from 192k to 48k did you do all that in the Windows Sound Control Panel?
Set what to 100% in the Levels tab? I can't set the overall Speakers / Headphones to 100% as then I'd go deaf.Go back in to the Sound Control Panel & check the levels tab. Set it to 100% if it's not already there.
I changed to Stereo earlier, that's one way I tried to fix it.So you're using surround? You're only capturing stereo with OBS so it's getting downmixed. Turn surround off & just use stereo.
I don't ever remember changing it back to anything else. I'll change it back to Stereo again even if it already is in Stereo and try another recording.Switch it back, your levels should be better & the echo should be eliminated. I think your getting the echo from the surround channels as generally they are sightly delayed from the Front & Center channels.
