I'm been having a problem for a while now and I haven't found anyone else who has this same problem.
Relevant info:
Windows 8 64-bit
OBS 64-bit (windows version, of course)
For recording videos, I use nVidia NVENC with 12000kb/s bitrate and CBR.
For streaming, I use x264 with 3500kb/s bitrate and CBR.
I record/stream in 1280x720 at 60fps.
Advanced options for recording: NVENC preset: High performance low latency. Encoding profile: high. Keyframe interval: 1. Use CFR: yes.
When I record or stream with OBS, it always says FPS: 60 and Dropped frames: 0. When I play the video back in any media player or edit it in Sony Vegas, it's very obvious that the video is not 60fps most of the time. This happens with both x264 and NVENC, and I've tried changed numerous settings, but nothing has helped.
And yes, I do get over 60 fps in-game.

The log analysis says my audio and video bitrates are excessive for streaming, which I already know, I was recording a video, so I used higher bitrates.
The only thing in the log analysis that is interesting is that it says I'm using a slower preset (high performance, low latency) but isn't the point of HPLL to be faster?
Any help would be appreciated!
