When I try to record in Mp4, it comes out as an mp4 file but when I open it, only the audio seems to be recorded and not my screen, any help?

What are you using to open the file? Some video editing programs don't like how OBS handles framerate.

Also, you really should record to FLV and transmux to MP4 if needed. OBS seems to have a high-CPU bug when recording locally to MP4 for long streams that can turn into a close-corruption bug. Also, if OBS crashes at any point, an MP4 will be entirely unrecoverable, while an FLV should only lose a bit off the end at most. Just change the filename to .flv in the settings, and it'll record that way.
