OBS lost recording

OBS was apparently happily recording a 3 hour zoom conference this morning / afternoon.
Conference over, stop recording button pushed, fine.
Went to edit, found only about 20 minutes.
Log file attached.
This seems the interesting part:

Don't know what it means , but don't see this on successful recordings.
The host system is windows 11, recent security patch done today, wasn't busy doing anything else, tons of resources (terabytes of free disk space)
Yes, it's recording to mkv.
What happened?
Is it recoverable?
How can I make sure this doesn't happen again?
