Question / Help OBS and game crashing when I try to preview

Hi guys, I'm new here. Because of the character limitation, I'm going to have to break this up into multiple posts.
Trying to start a Twitch stream but having issues.
At first when I'd have OBS and the game window open, OBS would crash when I pressed 'Preview Stream'.
You can view the most recent crash report for that below:
Code:

reproduce this crash, please submit this crash report on the forums at
http://www.obsproject.com/ - include the contents of this crash log and the
minidump .dmp file (if available) as well as your regular OBS log files and
a description of what you were doing at the time of the crash.

This crash appears to have occured in the 'c:\windows\system32\nvwgf2um.dll' module.

After this crashing I changed a few things.

The website suggested analyzed my crash log file and said the following:
"Multiple video cards detected

If you are having problems and are using a laptop, see this thread for advice.

If you are using SLI or Crossfire, you may need to disable it for compatibility with OBS.

Hook conflict detected

Game capture detected a hook conflict but was unable to determine which application was responsible. Try closing any other applications that may be injecting into your game, such as FRAPS, D3DGear, MSI Afterburner, AMD Raptr / Gaming Evolved, Action!, TeamSpeak/Mumble Overlays or any other kind of FPS monitor or overlay program

Bad resolution / FPS / bitrate combination

The combination of settings you are using will likely lead to a low quality stream with artifacting and other problems. Consider either raising your bitrate or lowering your resolution / FPS (qval: 0.032)

3 issues found (2 major, 1 minor)."

So I went to the Laptop thread and tried what they suggested. I also lowered my stream resolution and FPS a little bit. As far as I'm aware, I don't have any of the listed programmes that might cause the Hook Conflict. :S

Anyway, now the game window itself crashes when I press 'preview'.
