Looks like this:
Error: Texture->Map failed: 0x887a0005 0x887a0007

Your video card or driver froze and was reset. Please check for possible hardware / driver issues. This error can also occur if you have enabled opencl in x264 custom settings.
With the newest Nvidia Driver, 347.09.

System:
i7 5820k OC 4,5GHz @ 1,26V
GTX 970 Windforce 3x from Gigabyte, OC (+202Mhz GPU, +293 VRAM)
ASUS X99-A Board, 16GB Crucial DDR4 2400 CL9 RAM (xmp profile, no OC).
Windows 8.1 latest updates installed.
And there are 3 monitors and a TV on the GPU, so this might be a point, but i expect most people who stream have at least 2 monitors^^

Latest OBS. Log included.
x264, shadowplay disabled, no injection in the game (like MSI/Riva or Fraps for FPS counter).

The error happened with DayZ running, not tested with other games, but LoL crashed my driver without OBS.
I tryed to roll back any OC from my GPU, that didnt help at all.
The error was random and not reproduceable, but it occured with streaming or a simple preview. And the biggest problem was - the driver crashed hard, sometimes OBS crashed too and in some cases the driver would not restore at all, so the only option was a hard reset, not really what you want while streaming.

Right now i use the 344.75 driver and so far its working fine.
I still dont know what it was, but i think DayZ has something to do with it, the unoptimized piece of crap has more bugs & glitches than any other game i know and the worst performance at the same time.
So anyone who gets this error - get an older, stable driver, do a clean install, get an older OBS version if that doesnt help.

And the last option was - the system is not clean, there is so much shit because i use the old windows, moved from AMD GPU and Z87-Board to Nvidia & X99-Board, so older driver may still sit in the registry and cause problems. I will install a clean windows and try it again.

But this happens only with OBS and 1 other game and its random. Was perfectly stable without OBS running... Nvidia doesnt know anything about it and they told me to ask here..

Old driver working and newer driver not seems pretty conclusive that it's a driver problem. OBS asks the driver to map a texture and it crashes, not much we can do about that.

Well i would reinstall windows and get rid of the old junk that may cause problems. Then i will check it out, it happened in preview too so i dont have to go online. Or i will wait for the next driver release, that can take a while because Nvidia... they just dont do it very often^^
