Need Help with Twitch Layout

I'm streaming on Twitch and since my name is "JoJo", I had the idea of making a follow/host alert with the "To Be Continued" meme. I know how to make an alert en stuff. So my only problem is making the stream freeze and put a "To be continued" filter when I switch a scene or press a certain button.
Does anyone have some ideas or could tell me if this is even possible.
