Bug Report "Map Crash" on Stream Restart

When I start up OBS and then start a stream everything works fine. After some time I can stop the stream and OBS is still running. But when I then try to start the stream again it gives me the "Texture -> Map failed 0x887a0005 0x887a0020" error which usually is a driver problem.

I don't think this is a driver problem though - I tried it several times - everytime the same - crashes on second stream start. When I try closing OBS after I stopped the first stream - OBS freezes.
Also tried it without the installed plugins (CLRHost with CLRBrowser and Countdown).

Specs:
CPU: i7-4770k OC @4GHz
RAM: 16GB 1600Mhz DDR3
GPU: GTX980 (no OC - extra voltage +87mV and extra power 125% with MSI Afterburner to make it more stable)
GPU Driver: Newest 352.86
Power Supply: Corsair 1kW
CPU&GPU&Motherboard watercooled ~30 - 40°C
OBS Puffer on HDD 2TB - 5700RPM

Try older driver versions or underclocking the GPU.

Tried rolling back to several old driver versions now - including the first driver ever released for the GTX980.
Also tried underclocking my GPU. Still the same issue.
 
Unfortunately your GPU may be defective. If possible, try with a different one. It's really hard to pinpoint this problem, basically OBS issued a pretty boring command to DirectX (map a texture), and DirectX reported that the GPU failed. There's lots of possible reasons, from unstable drivers, overheating, bad GPU memory, unstable power supply down to simply a bad GPU that can't handle the load OBS and game puts on it.
