Question / Help Keep losing stream, help plz

Maybe I have got setting wrong or something. I will be live streaming on twitch; twitch says things are good on my account, but people viewing say the streaming stop. i have livestream three times and each time it has gotten worse. last stream it happen five times during the stream, and i lost my viewers, very sad. the first time i streamed it only happen once, it happen an hour into the stream. I am just learning this streaming, and I am not sure on the settings, that is why i think might be settings problem. on twitch, i watch my past boardcasts, there also time you hear my voice, but no video. I am using obs to stream.

I dont know what doing wrong, i hit the code button, but it keeps coming out like it is.
This is what get 'Error- unable to get back buffer from Swap Chain'
I don't know what Swap Chain is. I am very new to all of this live streaming, and it has be years since I have done computers or websites.
I am feel stupid right now, very sorry.

C:\Users\<username>\AppData\Roaming\OBS\logs. when I tried this, my windows said it cannot find. I type in my username. don't why.
