I've tried a lot of stuff and can't seem to get my videos to look smooth. I had it for a couple minutes, change a setting to get it not to lag and it went back to this spotting thing. Any ideas?
Can you please post a log from a normal recording session? We need a log file of a longer session to spot any potential problems you might have later in a session.
Oh, your bitrate is entirely too low. NVENC doesn't handle 0 buffer size the same way x264 does for high quality local recordings, I think. You need to specifically set both.
