Ghost Audio - Replay from previous recording

Hello, when I open OBS Studio, it immediately starts playing the audio from a video I recorded a while back. I've uninstalled the software and reinstalled it, I've moved the video and deleted it, yet it continues to play the audio from this recording the instant i open the software. Making recording anything new impossible.

Any help is appreciated!

We have a client who encountered the same scenario yesterday. Across thousands of broadcasts, this is the first time we have seen this. Client is broadcasting using a laptop attached to a wired Ethernet connection. Closing OBS, removing it from Task Manager and rebooting seemed to fix it but not sure why it happened or how to implement a permanent fix. Any guidance is greatly appreciated.
 
We have now replaced everything in the chain, hooked the streaming PC up to wired ethernet, replaced all cables factory made and we still have the same problem when switching camera/audio inputs back and forth in OBS.

The OBS log shows lagging audio and when monitoring it there is a studder. If I remove OBS from the equation, the mics and the loop through the PTZ optics work fine all day long.
 
I started having the ghost audio appear two updates ago. The audio plays through my output device (headphones) but does not print to the recording or stream. I have been able to open Task Manager and locate the offending task and end the task. This ended the audio and didn't impact the current running OBS session. Since the latest Windows 11 update I no longer can find the OBS task that causes the ghost audio. see the attached log files.
