Encoder initilization failed You can not take video gives this error

A forum search indicates that QSVHelper.exe problems have to do with the Intel Quicksync drivers crashing. Try updating your Intel Graphics drivers. If you're on Windows 7, then do a forum search, there's a specific driver release (not the latest either) that is semi-needed for it to work right.
