According to your log, you're still running on the Intel card. Are you sure you set both OBS.exe fiels (32bit and 64bit) to be run on the nVidia card? Also, in OBS itself, where it gives you an option to choose a video adapter, make sure you pick the top one, regardless of what the label actually says.

Under both Program Files and Program Files (x86), OBS has a folder. I made sure both of the applications were using global setting for the Nvidia. In OBS the top video adapter is selected. I also made sure the games im running are using global setting. I use the monitor capture for my source. I have reinstalled OBS twice and made sure the OBS folders were deleted in Program Files before reinstalling. Could this be a windows 8.1 issue? Here is the updated log:

Reinstalling OBS doesn't do anything. Deleting its appdata directory is what resets its data.

As for your problem, OBS is loading up on your intel:
18:43:01: Loading up D3D10 on Intel(R) HD Graphics 4000...

If you are trying to capture a game, you will need to go to the nvidia control panel, and set OBS.exe to use the nvidia instead from the nvidia control panel itself (do not set the adapter in OBS).

It seems in your second attempt in that log, it loaded up OBS on the nvidia. However, it seems more like you selected the nvidia in OBS' video adapter selector, which you're not actually supposed to do for laptops. For laptops, the top item of that adapter drop-down box (whatever it may be, intel or nvidia) must be selected in OBS. Then, you have to use nvidia's control panel, accessible via windows control panel, to actually have it set OBS' video adapter.

Yeah I did that and its still not working. I know what I'm doing is right which is why I posted this AFTER reading the forum for this issue. So everything in that forum I have done properly. I havent messed with the OBS settings at all, only to make sure the top adapter is selected. Ive checked everything in the Nvidia control panel and all the settings are right.

I've been over this with users many times, almost always it's a misconfiguration. I actually used teamviewer so much with users on this one issue that teamviewer banned me because they thought I was using it for commercial support purposes (jerks). When I did have access it was *always* a misconfiguration that caused the blackscreen if it was an nvidia laptop. So either you have it configured wrong, or have a new type of system I haven't seen before. You could try messing with the drop-down in OBS I guess as a secondary option but I doubt it'll change anything, if it doesn't work set it back to the top item.
