First, change your base resolution in OBS' video settings to 1280x720. Second, make the video capture source fit to it, Ctrl-F or Ctrl-R rather to reset.

Generally speaking, more bitrate means more quality, no more than the max bitrate they want is typically 3500 on twitch.
 
The elgato only runs at 720p. If you want the best quality, you want your stream to operate at a 1:1 pixel ratio with what you're capturing.
 
The Elgato is capturing at 720p, and your base resolution is 720p, so it should fit by itself. If not, then yes, Fit To Screen should make it fit, as should "Reset".
