If you need to increase your desktop audio levels past 100%, you can use "desktop audio boost" in audio settings, which multiplies the signal by the numbered entered (up to 20x the volume)

I have that setting at 20 , Still cant hear almost anything when I stream

Everything in my PC is at 100% Volume , Any thing else I can do?
This started happening just about 2 weeks ago , I do not have the old logs do...

Post a vod of the issue. If it's at 20x and it's not working, then there's some configuration with windows or your computer that's not-so-good. This definitely isn't something that would be 'caused' by obs itself. OBS just captures the sound as-is. It doesn't do anything to its original levels unless you use the volume control on the main window or the audio boosts in audio settings. Everything else is on windows' end or the device's end.

Very strange indeed, I saw you mentioned everything is on 100% volume, I assume you also checked the volume mixer and its setting for OBS:

If nothing helps, I personally would remove my realtek drivers and try reinstalling them ;/ oh and do you see visual feedback in OBS:
