Audio issues since windows update

OBS has been great. New update on windows showed up a few days ago and i went to stream for the first time today. If Realtek or default are selected and i have my headset plugged in, no sound gets outputted to my stream. There is a new sound option in windows “headphones” that shows up now. If that is selected, all you hear on stream is a screeching mess. Back when obs worked, even with headphones plugged in, the only option was Realtek speakers and it worked. Anyone else having this issue and if so any ideas on a fix in an upcoming patch or temporary work around? Thanks so much for any help.

The joy of Microsoft and desktop (Win10 and newer) as a service... meaning functional changes not limited to Service Packs... but I digress, sort of ...

Specifically, I haven't had this issue, but
- have you checked the Windows OS audio system (new System Settings and the older Control Panel applet) to check Audio devices, and security permissions for those device?
