Audio is doubled or tripled when I stream, please help and assist.

Good afternoon everyone. I do not consider myself a tech guy so I hope someone here can help me. I recently got a new computer this past halloween. My old computer was out of date. I had run OBS and streamed when I could but didnt have an issue for the most part. But nothing seriously. Anyway, I have been trying to get everything up to date when I got a new computer. But everytime I stream, I hear what I like to call PA Announcements in both my voice and the the screen capture. Btw, I use an HD60x card off another TV, not a computer monitor. I do not play on PC (but I hope to do so at a later time). But I do stream PS5 games. Anyway, things I have tried is messing with the audio settings as just have a blue yeti mic (I think its something related to this but not sure). I have done advance audio properties on all audio mixers (monitor is off on everything). I have tried adding, deleting, and readding the sources. I have also deinstalled an reinstalled OBS. I also messed with the device manager of disabling and reenabled the different sounds. Finally I messed with the sound settings of turning off and on and muting different sounds. But nothing seems to work that I know of. I can hear youtube videos and things just fine also with no extra sound. Its just really frustrating.

Finally and I dont know if this has any bearing on things, I have like a sports bot (its just a place where it shows sports scores like NFL, NHL, etc). It was on the old computer but never added it to a source on the new computer. It still shows up in the stream though. An example is:
Sport_Scores_Bot
: [NHL] Philadelphia Flyers Goal Scored! Tied with the Oilers 1-1 with 04:22 left in the 2nd period

Is this something where I just need to go back to the original, old computer and delete OBS there (It was an older version too btw) or is it something else entirely? Sorry for the long post everyone, I just am really stumped on this as I have tried for about 3 weeks to figure this out. Thanks for reading and any advice will be appreciated.

Are you using a headset, or speakers?
If that mic is picking up your speakers (and that audio is already captured separately), then you can easily have an audio loop/echo.

@Lawrence_SoCal : No Headset. Blue Yeti Microphone, HD Pro C920 Webcam, Elgato HC60X Capture Card. (Only the microphone is on in the Audio settings):
