Hm, I need a different log file if that's okay. Instead of giving me the file, use the help menu. Your logs can be accessed in the application via the Help menu ->Log Files menu item. Give me the last log file where this happened. I definitely want to figure out what's going on here and what sort of system you have because this is very unusual.

Thanks, Jim. I believe this is what you're looking for? I've used OBS Log Analyzer and this is what I'm given: link

Okay, here's what I want you to do, first, I want you to set your audio to 44.1khz in encoder settings. Then try OBS again.

Then, if that doesn't help, I want you to try the latest test build: (older test build, download 0.631+) -- We've changed things a little bit with audio in it. See if this helps at all (though I'm not sure if it will).

Third, if neither of those things work, I want you to try this:
1.) Open a command prompt as admin
2.) bcdedit /set useplatformclock true
3.) Restart the computer

and then let me know if that helps at all

I've tested 44.1kHz with OBS v0.625b, but the audio/video starts off desynced.

I then tested 48kHz with OBS v0.63b (test build 1), and there are no issues.

Just to be sure, I will test 44.1kHz with OBS v0.625b (test build 1) the next time I stream. I will then post results here.

Thanks for the help, Jim!

This seems to be a problem on your end as the code hasn't changed in the test builds.
