Not sure if this is an OBS or QSV bug, but I happened to run across it when changing my bit rate from 55,000 to 70,000 when deciding to bump up from 30-60 FPS (I seem to remember previously using 80,000 without issue, but I may be mistaken).
Setting it to 70,000 kbps outputs ~4,400 kbps for some reason (higher than 70,000 and it seemed to start getting random).
Settings attached as well as 1920x1080 @ 60 FPS, QSV (CBR w/checkboxes to use global settings), QSV preset 1(best quality), profile high, i5 3570k.
Just an FYI I guess, I've now changed over to 22 CQP @ 65,000 kbps maximum and 1080p @ 60 FPS seems to look fine.
