Question / Help 2 Sounds coming through stream?

Every time I talk, in the twitch stream it sends 2 sounds through. It repeats what i say twice as the same with in game sound. Please help!
 
Post a logfile. We really need it to be able to help. Why it asks for one when a new thread is opened in Q&H.
