Question / Help 2 Bugs: "Encoder initialization fail" & Won't record video, only audio.

Okay, so first the "Won't record video, only audio" issue. This has happened ever since I started trying to record with OBS;
It would record just fine. But when I go into Windows Media Player to check out my video. There is NO VIDEO, but I can hear audio just fine.

And with the "Encoder Initialization fail" Error, I don't know what to say.
Also, I included the Logs.rar because I do not know which log is the right one, seeing as I failed and tried, failed and tried.
My CPU: Intel Core i7 4790K
My GPU: Nvidia GeForce GTX 980

PLEASE HELP!

Probably some Windows 10 problem. Are you audio and video drivers updated as much as you can for the new OS? Can you post a small sample file to a file sharing site? Although if Encoder Initialization Failed, I expect there to be things missing, like all the video. So probably goes back to a Windows 10 problem.

It would work for a little bit, but maybe I should just re-install it. Probably just missing files. But then we get back to the only recording audio issue. I'll test this theory.

Hold the phone, I wasn't using the x264 Encoder!

Okay, I fixed the "Error Initialization Fail". This was due to the Nvidia NVENC setting done in the encoding section.
But, the "Not recording screen, only audio" issue. Has not, however whenever I preview - record. The OBS Application shows that its recording and all is well. But when I go into Windows Media Player, or any application to view video. Or even Sony Vegas.
I will send some screenshots of my settings.
My settings were took from this reddit post; https://www.reddit.com/r/gamedev/comments/21ga7r/tutorial_how_to_make_lossless_high_framerate/
I did these settings to get better FPS/Quality with Small File Sizes. I doubt the settings are the issue. It's probably Windows 10.
