Code:
12:57:19.738:
12:57:19.738: ================================
12:57:19.739: Warning: OBS is already running!
12:57:19.739: ================================
12:57:19.739:
12:57:22.889: User is now running a secondary instance of OBS!

First make sure to only have one instance of OBS open. More than one instance might conflict with each other.

Code:
13:10:55.100: [game-capture: 'minecraft'] attempting to hook process: javaw.exe
13:10:55.110: [game-capture: 'minecraft'] using helper (compatibility hook)

What the previous user meant was to turn off "Use anti-cheat compatibility hook" in the game capture properties.

Another suggestion is that since you have a multi-GPU mobile setup, you might want to force both the game and OBS onto the same GPU: https://community.amd.com/docs/DOC-1581 . Without doing this, you may not be able to capture the game as you intended to.

Does this happen with any other game or is it just Minecraft? Also is your GPU Driver up to date?

You can get up to date GPU drivers from AMD here: http://support.amd.com/en-us/download . Just run the auto-detection, it should be able to give you the correct drivers. If not you'll have to manually find the driver by selecting the correct values in the lists and clicking Display Results (Hint: You have Windows 10 64-bit, an A-Series APU with HD 7000 graphics and a mobile HD 7600M GPU).

Usually these kinds of things should be solved by having both the game and OBS running on the same GPU (this link shows you how to do that: https://community.amd.com/docs/DOC-1581)

Fenrir mentioned in IRC that OBS is hooking D3D9, but Minecraft is an OpenGL game. Make sure that you do not have any conflicting software installed (RivaTuner, MSI Afterburner, overlays, ...) that could cause this.
