Where's my show/hide toggle in source list?

I believe I lost access to show/hide toggle (and the lock toggle for that matter) in my sources list. I'm on OBS 30.0.2 and I don't believe I have any plugins.

Image below with source list stretched to max horizontal length. What can I do to get these toggles back in my sources list?

UI theme files are damaged or inaccessible in other way. You can try to check integrity of OBS files in main menu Help > Check File Integrity
Also, you may try to to switch to other UI theme in main menu File > Settings > General > General group > Theme
