Try to specify different Video Format properties manually ('Video Capture Device' source, OBS Studio) - you need to set Resolution/FPS Type to custom and setup resolution (640x480 or lower) first.

It's a fresh windows install and it's updates, has nothing but hearthstone, drivers and OBS

I don't have more video formats outputs on this particular webcam, and I use even lower res than 640

Id try it on another pc, and another cam on your pc

I think, your camera hasn't proper DirectShow support or you are missing some driver/filter.
1. Can you find model of your camera?
2. Camera has build-in mic?
I'm second to Mroczny_Gustaw, try different system (7,8, both x86 and x64).

I have come to the same problem, how did you solve the problem finally?
