Two USB video sources won't work?

I'm trying to run a stream using two different USB video sources (one a PS4 and one a camera connected via HDMI cable) but when I try to add the second one to a scene it fails to render in preview or window? Do I need to have different adapters for both?

That depends on the USB capture device(s) you're using. Some of the cheaper ones' firmware cannot handle enumerating multiple of the same device. You listed your sources, but not the important part... the actual capture device(s) you're using, or a logfile.
