Stream goes offline, often while changing scenes

I've been having this problem more and more lately.

My OBS will randomly go offline while streaming, the "Stop Streaming" button gets stuck and I have to relaunch OBS.

It usually seems to happen when I switch games, update my stream info, or just switch scenes.

Attached are some logs from today.
