Sound + Video starts synced and gets worse over time

Hiya. I have a problem that I am at my limit with trying to figure out. I used to be able to watch my OBS preview window and play my Switch for hours, and it would never desync. Or lag. But lately, it has started doing exactly that. It will start just fine, but over time will desync. What's even weirder is that the desync varies. Almost like it can't keep up? Sometimes there is a lot of desync (maybe half or a full a second), but other times there is only a little desync (noticeable really only when scrolling lot a menu quickly). Also, to further complicate the issue, it seems that sometimes the issue doesn't happen at all. Its almost like it depends on how my PC starts up?

I have always had the audio dsync, I have posted here a few times about it; never able to find a resolution. This might be the same problem, not sure. Its totally possible I never noticed the video desync portion (its been a bit since I have played by watching my OBS preview window). The only change I have done recently was I added a 3rd monitor, connected to my Graphics card. Its a cheap acer, K202HQL.

I am at my wits end. Open to trying anything, but my only request is that you provide an explanation with your explanation so I can learn why it might matter.

Set up: Nintendo Switch > Powered HDMI Switch > El Gato HD 60 Pro via HDMI > OBS. I have attached my most recent Log File also.

Oh, things I have checked or tried:

I have verified that my sampling rates are all the same in OBS and the Windows Sound Menu.
I have looked through my settings to see if any of my settings might be causing the issue.
Done a ton of searching online read a ton of threads; couldn't seem to find a solution that worked.
