I've looked for help numerous times and have found nothing that has helped me. A few months ago I could stream perfectly fine (twitch.tv) but then deleted OBS and the settings along with it. When I started to stream again it's always buffering and lagging, but in game I always get 60+ FPS. No parts have changed in my PC, and all my drivers are up to date.

League of Legends

It contains all of your settings, screenshots aren't needed. It also contains a lot of information from the back-end about what's going on with your machine, that we need to be able to spot any problems and offer possible solutions.
 
Looks like your video card is having a major issue; 50ms on the video thread is ridiculous. It should normally be under 5.
i5s also tend to run out of gas much past 720p@30fps. Your CPU thread times aren't great, but wouldn't cause a 90%+ late frame count.

Looks either like a GPU driver update issue, game update issue, or something to do with the captures you're using. After all, just because the hardware is the same doesn't mean the software hasn't changed. But something is REALLY wrong. I'd recommend a full clean-uninstall of your GPU drivers, and installing the latest fresh as a first step. I constantly ran into problems if I tried to install new over the existing ones, when I was on my Radeon 6870.
