Reflector 4 Issue With OBS

So I just downloaded Reflector 4 so I could stream games from my phone. I pay for it, download it, look up I to set it up, and when I go to boot up the capture screen it's just....not there. Like you can see a bunch of arrow pointers and plus signs but it's on a black screen. I don't know what this means or how to fix this. I am brand new to this stuff so very glad I have found a forum where someone jumps in promptly to provide assistance. Although I am not the OP I think I have the same problem. I followed the advice from qhobbes above as far as I could but I was confused when I looked at the .exe file Advanced properties because there is more than one checkbox and none of the checkbox descriptions sound like they would have anything to do with the problem. In any event updates were downloaded and applied, as you will see in the attached screen shot I am now using OBS 1.3.2 - I do not know what the version was before. Before the updates were applied I was seeing in the window at the top of the OBS window a green box which seemed to have the right size and shape for an iPad screen and the box could be moved around but it is empty. It did not contain what I see in Reflector 4 (the Reflector 4 window also is in the screen shot - upper right corner. From a number of Youtube videos I have watched the preview window at the top always shows the actual content of the camera or screen.

Further help from qhobbes or anyone else would sure be appreciated.
