Question / Help Pixelation/interlacing only on stream

I am new to streaming and we are having an issue with some visual artifacts (sample attached). For any fast motion, it appears as tearing/interlacing/pixelation (I'm not even sure what term is appropriate). However, this video artifact only appears on the stream (the Projection view from the Program and Preview windows do not display this artifact - neither does a local recording). We are streaming at 720p w/ 4Mbps up stream, using Hardware NVENC. Any suggestions?

(I'm sorry that I don't have access to the OBS log file currently - will try to add it if I can)
