Bug Report OBS won't record or stream.

Yet another problem with this damn program. Now everytime that I try to record, it won't do it, saying the output failed. I have the window I want to record open, I've got nothing else running, I've got the settings all set, SO WHAT IS IT'S PROBLEM NOW?!?!
