OBS scrambling video output

I am not sure what to call this, or even what to search for on it. I am attaching an image of what happens. For some reason, whether streaming or not streaming, the preview video screen(s) show a scrambled video (not pixelated, just scrambled). If this happens while streaming / recording, that is what goes out to the live stream / recording. Still images don't seem to be affected. The left hand preview window looks normal because there is a still image there - if a video was playing there, it would be doing the same thing.

If anyone has any ideas as to why this keeps happening, we would be very grateful! :)

OBS version: 25.0.0 (have tried the latest version with the same results - we went back to 25.0.0 because we thought it hadn't been doing it since then). I should also note, that yesterday, I formatted the drive and reinstalled Windows 10 on it from scratch, and setup OBS from scratch (no config was copied/restored). Also not using any plugins in OBS. That did not fix the issue.

Image attached. Logs attached (the one time stamped 13-58 is the most recent one - so far this anomaly had not occurred, the one from 13-22 should be a log file from when this happened).

Oh, just realized, the bottom part of the left hand preview has a video capture device in there, that is why that is also messed up, but the image, as you can see, seems fine.

We've ensured that we have the latest video drivers installed. Could this be a video codec thing?

In case anyone is reading this :)

Just had a thought...my scenes that contain videos also have a capture device for our camera in the scene as well. I don't suppose that could cause a problem...camera is behind the video, that way when the video is done, it automatically goes back to camera....

I have seen this same issue a couple times now while streaming live sports. The video capture device is garbled in any number of ways as the OP states. Static scenes are fine. Scoreboard overlays on the video capture scene are also fine. Shutting down and restarting OBS resolves the issue. I haven't found a way yet to resolve while keeping the stream up.
