OBS recordings have audio errors (loud squeeks randomly)

Hello, I come in dear need of some assistance.

I've been having audio issues for a long time now and since I'm trying to get back to content creation I realized one of the reasons why I stopped was because of audio issues.

I thought it was purely a bug with the AMD platform and I was kinda screwed, so I updated the BIOS, upgraded my CPU to a 5950x to alleviate any possible bottlenecks and my old 2700 had a few hiccups with being overloaded, the audio issues are still present. Turned off all overclocks. I went through all my sound devices, did the normal downgrade of 48000 Hz to 44100 Hz, disable exclusive mode, disable all enhancements (which non were on). Unhooked Voice Meeter and went stock, recordings still odd. Downloaded LatnencyMon 7.00 to see any possible issues, non. Looked for any WHEA errors, clean as can be.

Eventually, I figured it was directly related to OBS, though a while back I remember odd occurrences watching random videos having issues but it was so rare I didn't take notice. So I changed the codec, changed the sample rate to 44.1 kHz. Still, issues are present and my CPU wasn't overloaded.

Literally been searching all over, from hardware stuff first, all the way down to software and any applications involved. Now I'm kinda at a dead road scratching my head full of frustration, I please ask any person who knows how to solve this issue to lend a helping hand. Here are some logs from my most recent recordings which had issues.

Your most recent logfile shows that your Quadcast is running at 48kHz, while your other sources are at 44.1kHz. That sampling rate mismatch could definitely cause it. Likewise, Voicemeeter can screw up and cause processing issues depending on configuration (which is out-of-scope for the OBS support forums, as third-party software). All of your logs show VM in-use.
All three logs show OBS running in 44.1kHz sampling mode. In all three logs there is at least one 48kHz source.

Your first logfile also shows some SIGNIFICANT issues that should be corrected ASAP.
You have multiple Monitor Captures pointing at the same display. They can and WILL interfere with each other. Create one, then use 'add existing'. Delete the second set. One is called 'Screen Capture' (in one scene) and the other is called 'Display Capture' (in two scenes).

You have Monitor Captures in the same scenes as Game and Window captures. Monitor Captures should be avoided at all costs. If you need to use one, it should be in its own separate dedicated scene. They again can and will interfere and cause performance issues when in the same scene as a Game or Window capture.

I removed the program entirely and did 2 separate recordings, both GPU and CPU methods resulted in audio squeaking randomly.
(Note I didn't use my mic for any of the recordings (even in the logs for the OP) it's just a raw recording of gameplay)

Also, MP4/MOV/TS are not recording-safe formats and should NEVER be recorded to directly. Use MKV or FLV.

Thanks for the help although no issues were tied to my mic/ game/ obs, the formats weren't the issue either.

apparently, the hard drive I was using had issues with audio playback BUT NO ERRORS. I don't know why, I updated my chipset drivers and it dawned on me that it could be a storage issue. Sorry for wasting your time, but I appreciate the source tips.
