If you have fast upload: Right click obs.exe in task manager and choose create dump, zip it and upload it to http://www.copy.com and PM the link to me.
Your crash dump seems to show that OBS is freezing when trying to initialize audio. What audio devices are you using? Check your default playback and recording devices and make sure they aren't in use by other programs and that any 3rd party audio extensions / filters are disabled.
I experienced a similar problem. I messed with every setting I could think of. I changed my webcam from 1920x1080 to 640x480. Every time I started to stream or hit preview it would freeze for 10 sec+. What fixed it for me was changing the FPS to 30 from 60.
