I am having a similar issue.
My webcam/game drops frames like their greased with butter.
I've tried system restore, unplugging the webcam into different ports, uninstalling obs & the webcam and reinstalling, virus scans, update scans, and ever combination of OBS settings possible and some times the cam with display properly, but as soon as you hit record it starts dropping.
I think the latest windows defense update and OBS are having an argument as it were.
We may have to wait for updates/patches sadly.
(my webcam (logitech c920) works in it's own software, but no where else. Best clue I can give rn.)
You are not alone with this issue.
Something is more wrong here than just user settings.

USE THESE SETTINGS

Also keep in mind, these are my settings, and I am on a pretty powerful system, so work around to yours
I have an I7 8700K
w/ GTX 1070 SLI using a dedicated GPU for rendering/encoding to keep framerates high.

OUTPUT:
Encoder: x264
Enforce streaming service encoder settings: CHECK
Rescale output: 1280x720: CHECK
Rate control: CBR
Bitrate: 3500~4500 (depending on internet speed, I recommend in between these 2)
Use custom buffer size: CHECK
Buffer size: 3500~6000 (also depending on internet//overall PC performance)
Keyframe interval: 0 (will likely auto to 2, but keep it at 0 if it auto's to something else.)
CPU Preset: MEDIUM (do not use anything lower than medium I.E slow//slower)
Profile: HIGH
Tune: either none, fast decode or zerolatency

Now go to VIDEO SETTINGS:
Base canvas: 1280x720
Output resolution: 1280x720
Downscale filter: bilinear//lanczos
Integer fps value: 80~90 (do not use common FPS value as it will not keep a stable FPS of 60 true, it will be going for 60, but usually it will AVG at 59.98 instead of 60 true, thus raising the fps the stream will look buttery smooth, this was my problem that I was looking for until I just recently found it out today by myself.)
