I am having this issue too and all my research points to a glitch in Windows 10.
Does the framing issue happen while OBS is not running?

Your webcam output doesn't need to take over the full screen.
Reduce it to 640x480 to cut down on video processing. With that CPU of yours, it needs all the help it can get.

Fortnite is fixed because you set a frame limiter on it in the video settings and you are likely hitting that as a capped limit. That is perfectly fine. Also, in various parts of the program, there are fixed frame rates. For example, the lobby which is set to do 60FPS.

You are running two monitors, right? I see two output ports:
18:16:03.966: output 1: pos={0, 0}, size={1920, 1080}, attached=true
18:16:03.966: output 2: pos={1920, 0}, size={1920, 1080}, attached=true

Is one a 144hz and the other a slower 60Hz? If so, disable OBS preview. OBS has a problem running with two different monitor refresh rates.
