You probably have a crash report. It is needed also.
Please do a clean log. This mean, restart OBS, do as you normaly do streaming/recording for at least 30 seconds. Then upload that log.
Both logs (normal and crash) can be uploaded from the Help menu.
I saw you are using an external plugin. Remove it and test. Also, you are using a non standard aspect ratio (Video) wich can be unpredictable the result. Display and Game Capture sources interfere with each other, you need 2 separate scenes for each one to avoid possible issues.
If that don't work, try uninstalling OBS and reinstalling it. Make sure no external plugins are present. Test. Upload logs
