You clearly aren't given the attention it needs to the network issues you have. There's a reason why you enabled network optimizations and dynamic bitrates.
A log file with a test stream is needed to know more.
The capture interference is because you have put a display capture in the same scene with a game and/or window capture and you should put the display capture on it's own separated scene to avoid interference.
Enable Windows Game Mode as it helps.
If you used any network optimization software or manually changed it, undo all changes. This apps cause more issues than help.

Anything bellow 90 quality is bad and probably an issue within your ISP. It may be a bad cable or faulty router/modem. Try using another cable if you have an spare one, and in the router / modem change the port where the cable is connected. Test in between each change. If everything is fine, contact your ISP and tell them about the issue, and that you are trying to stream to Twitch, and that streaming uses RTMP protocol, online speedtest don't measure this protocol, but the TwitchTest do measure this.
If you can, post an image of the results of the Twitch test too.
