Bug Report obs crashing

worked once, now i just get this shit http://i50.tinypic.com/1zfmozo.jpg
any ideas how to fix?
-tried closing every n launching first
-all the settings are fined
streamed before just suddenly stops letting me stream comes up with above then closes,
for such a simple program its getting annoying.
-tried turning off stream view also.

so you're saying it actually did work, but this just started happening later at some point?

pretty much ive changed nothing streaming wise just went to load it up one day, exact same way and it just crashes when i click stream.
