OBS Crashing As Soon As I Hit "Record"

Im not sure why its happening. I recently restored my rig and redid all OBS settings, but I set everything up the same way and I never had this issue before.

Tried updating video card drivers & fixing issues that OBS help tool analyzer was showing, but no luck... Here is a new crash report:

The plugin you're using to record a source looks like crashing OBS.
Contact them about this. Show them the log file.
