Question / Help OBS crashes The Witcher 2

Hi, I just started using OBS two days ago and I've been loving it for streaming Hearthstone and other games. I bought The Witcher 2 in the recent Steam sale and tried to stream it, but the game crashes to the desktop before getting to the main menu (to be clear, I tested the game with OBS off and the game runs just fine). I'm not sure if this is a problem with TW2 or OBS or my computer (a quick Google search shows that others have had success streaming the game without issue), so I was hoping an expert here could help alleviate my issue.

Here are the relevant logs from my stream:

Monitor capture appears to work, however alt-tabbing out of the game and then trying to alt-tab in crashes it, which I doubt is an issue related to OBS.

I have no issue streaming the game in monitor capture, so I'll probably just do that from now on.

Yea, on windows 8+ monitor capture is great, so it's a decent workaround if something is conflicting with game capture.

Game capture conflicts (which cause crashes in games) are almost always caused by other applications interfering, such other capture applications (fraps/etc), overlay applications (teamspeak/mumble overlays), graphics enhancement and monitoring applications (sweetfx and sometimes applications such as msi afterburner depending on the settings).
