Question / Help OBS black screen- Trying to stream on ps4

Hello,

I am a new user and not very good when digging deep into the woods when it comes to programming and solutions.
When trying to stream I get the "Infamous Black Screen."
I have read and watched videos online but can't seem to get it going.
Is there anyone out there that can possibly help me?

I'm new to this computer stuff so I apologize if some doesn't make sense.
I have my PS4 HDMI running into the capture card and two Display port slots going to two separate monitors on the same card.
When I try to preview or stream on OBS it doesn't register anything and all I get is a black screen.
I'm also not 100% sure I have all the wiring connected properly so any advise would be greatly appreciated.

I'm going off of what the employee at Best Buy told me. He also brought up a splitter but changed his mind so I went with his word and assumed this would work.
