Looks like you need to update/reinstall the driver for the device.

Or place the device on a proper/supported USB3 port (i.e. a port coming from the Intel chipset, and not Assmedia crap.)

Drivers reinstalled (Extremecap U3, GPU and USB 3.0) - no effects.

My laptop have two 3.0 USB's. Testing with Avermedia diagnostic tool showing that everything is fine. Placing device on another 3.0 USB port has no positive effect. Still BSOD.

You might want to try updating the BIOS and chipset/USB drivers for your system. If that doesn't fix it then you'll need to file an issue with Avermedia. OBS can not cause BSODs per se. It only makes calls to DirectShow which then forwards them to the device driver, and the device driver is what is crashing Windows and giving the BSOD.

Do you get the same issue when you start the Avermedia application (sorry can't remember what it's called off the top of my head)

No. When I start Avermedia application (RECentral) - it is all fine! Windows crashes only when I trying to choose Resolution\framerate from Capture device properties dialog box in OBS. Otherway Extremecap U3 do not give any respond in OBS.

I've already send a message about this issue to Avermedia supprot, but they do not send me any answers yet.
There is no need to update BIOS and other drivers. They already updated. And as I said in message upper in AverMedia application (RECentral) there is no any problems.

Solved!

There was disabled source in OBS scene used Extremecap U3, tuned for translate from PS2 via component connection. Despite the fact that this source unit was disabled - only deleting this source allow me to solve BSOD problem.

Thanks to everybody for attention.
